AUTHORIZING EXT~SION OF HOSPITALIZATION
AND MEDICAL INSURANCE ~ITS TO PART
TIME OFFICIALS AND EMPL~ OF THE CITY
AND TO PROVIDE FOR PAYROLL DEDUCTION OF
PREMIUMS.
BE IT F~NACTED BY THE COUNCIL OF THE CITY OF FAIRLAWN:
Section 1: The Mayor/Director of Public Service is authorized to negotiated with
Dominion Life Assurance Company and to contract with said company for
extension of hospitalization and medical insurance benefits under the
group policy now held by the City. Said extension of benefits shall
be to such elected and appointed officials and employees of the City
as are part time officials or employees, provided that the cost of
such extension of coverage shall be entirely borne by the official or
employee covered, and that coverage shall be optional with each such
official and employee.
Section 2: Upon request of any employee or official electing coverage provided
in the foregoing section, the Finance Director may withhold the amount
of premium due from the earnings of each such official or employee, to
remit the same to the insurer as and when the same become due.
Notwithstanding any such payroll deduction authorization, the City and
the Finance Director shall not be liable for non- payment of premiums,
but shall account to the subject official or employee for all sums
withheld.
Section 3: This Ordinance shall be effective from and after thirty days after
its first publication by posting.
